• DBS Evidence Checker (Cathedral) Process Flowchart: a flowchart outlining the DBS process for Cathedral DBS Evidence Checkers.
• DBS Disclosure Cover Sheet (Cathedral): cover sheet to be completed by the Cathedral DBS Evidence Checkers and sent to the Diocesan DBS Coordinator.
• DBS Information Log (Cathedral): form filled in by Cathedral DBS Evidence Checker to record completion of the DBS check and track renewal.
